Performance Improvement: “Accredited CME as a Bridge to Quality” Stage B: Learning from the Application of Performance Improvement (PI) to Patient Care Implement an intervention based upon performance measure(s) selected in Stage A, using suitable tracking tools (e.g. reminders or flow sheets).
Step 1: Based on the information presented at the live CME activity you attended, and by reviewing the data you entered on the Data Collection Forms in Stage A, reflect on your current practice pattern and how it compares with the standards of practice presented at the educational conference. Step 2: Create and implement an intervention that will enable you to more consistently implement the optimal standard of practice specific to the performance measure selected in Stage A. Identify any barriers in the system and consider ways to overcome these barriers. The intervention may take the form of reminder systems and other downloadable materials or changes in SOPs or other system–based interventions. Step 3: Download and complete the forms for Stage B Step 4: Submit the completed forms to the Yale CME Office for Documentation. Keep a copy for your records.
